Timoptic

Timoptic (generic name: timolol maleate) is an ophthalmic solution used to treat elevated intraocular pressure (IOP) in patients with ocular hypertension or open-angle glaucoma. Comprehensive Drug Guide

How Does Timoptic Work?

Timoptic works by reducing the production of aqueous humor, the fluid within the eye. By decreasing the production of this fluid, Timoptic helps lower the intraocular pressure, which in turn reduces the risk of optic nerve damage and vision loss in glaucoma patients.

Common Dosages

  • 0.25% ophthalmic solution
  • 0.5% ophthalmic solution

Typical Dosing

  • For glaucoma and ocular hypertension: The typical dose is one drop of 0.25% or 0.5% in the affected eye(s) twice daily. Your healthcare provider may adjust the dosage based on your response.

Typical Dosing

FDA Approved Indications

  • Elevated intraocular pressure in patients with ocular hypertension or open-angle glaucoma

Who Shouldn't Take Timoptic?

  • Bronchial asthma or a history of asthma
  • Severe chronic obstructive pulmonary disease (COPD)
  • Bradycardia or heart block greater than first degree
  • Heart failure or other serious heart conditions
  • Known hypersensitivity to timolol or any of its components

Advice From The Pharmacist

  • Administer Timoptic as directed by your healthcare provider. Wash your hands before applying the drops, and avoid touching the dropper tip to any surface, including the eye, to prevent contamination.
  • Press on the corner of the eye (near the nose) for 1 to 2 minutes after administering the drops to reduce systemic absorption and minimize side effects.
  • Do not stop using Timoptic suddenly without consulting your doctor, as this can lead to a sudden increase in intraocular pressure.
  • If you are using more than one type of eye medication, wait at least 5 minutes between applications.

Side Effects of Timoptic

Common Side Effects

  • Burning or stinging in the eye
  • Blurred vision
  • Dry eyes
  • Headache

Uncommon/Severe Side Effects

  • Shortness of breath or wheezing
  • Bradycardia (slow heart rate)
  • Dizziness or fainting
  • Severe allergic reactions (rash, swelling, difficulty breathing)

Risks and Warnings of Timoptic

  • Respiratory Risks: Timoptic can worsen symptoms of asthma or COPD, leading to breathing difficulties. It should be avoided in patients with these conditions.
  • Cardiac Effects: Timoptic can slow the heart rate and may worsen symptoms in patients with heart failure or other heart conditions. Close monitoring is recommended.
  • Masking of Hypoglycemia: In diabetic patients, Timoptic may mask symptoms of low blood sugar (e.g., increased heart rate), so blood sugar levels should be monitored closely.

Interactions with Timoptic

Common Drug Interactions

  • Other beta-blockers: Using Timoptic with systemic beta-blockers (e.g., metoprolol or propranolol) may increase the risk of side effects, such as bradycardia or hypotension.
  • Calcium channel blockers: Combining Timoptic with calcium channel blockers (e.g., verapamil or diltiazem) can increase the risk of heart and blood pressure side effects.
  • Clonidine: Discontinuing clonidine while on Timoptic may increase the risk of rebound hypertension.

Alternatives to Timoptic

  • Latanoprost (Xalatan): A prostaglandin analog used to lower intraocular pressure in glaucoma.
  • Brimonidine (Alphagan): An alpha agonist used to reduce intraocular pressure.
  • Dorzolamide (Trusopt): A carbonic anhydrase inhibitor used for the same indications as Timoptic.
  • Betaxolol (Betoptic): A beta-blocker used to treat glaucoma and ocular hypertension.
